Case Manager

The Case Manager is responsible for professional clinical services and related tasks for individuals with mental illness, intellectual/developmental disability, serious emotional disturbances (SED), and/or substance use disorders. Services will be provided to children and/or adults in the community, home and/or virtually upon the person's served request. Minimally, clinical services provided will include: development of the person centered plan that assess, links, monitors, and coordinates services; assist person served and/or families with community resources; and coordinate services and maintain relationships with other professionals, team members, referral sources, community resources, etc. Maintaining effective quality clinical records is vital to achieve success in this position.
Qualifications and Education Minimum Requirements
Applicant must be qualified to be a CMHP with 3 years experience with a bachelor's degree or 1 year experience with a master's degree in the examination, evaluation, and treatment of minors and their families. State license as an LMSW, LLMSW, LMFT, LLMFT, LBSW, LLBSW, LPC, LLPC, TLLP, LLP, LP, RN, or a degree in a human service field from an accredited school. Human service fields can include, but not limited to Human Services, Psychology, Sociology, Criminal Justice, Social Work, Education, Chemical Dependence, Occupational Therapy, or Medical Case Management.
