Direct Care Worker

$500 sign on bonus after 90 days of full time employment!

The individual who fills this position demonstrates that he or she is capable of carrying out the following roles and responsibilities: 

  • Assist in transporting consumers to and from appointments
  • Maintain clinical records as designated by policies and procedures such as, but not limited to; documentation of physical observations, significant observation notes, and any behavioral management addressed with the accurate date and time services provided by staff at the end of each shift
  • Maintain accurate documentation for all purposes (Progress notes, data Sheets, Transportation log, etc.)
  • Orient consumers to treatment program
  • Arrange public transportation for consumers
  • Maintain clean condition of any assigned CSI vehicle, and report to supervisor any known maintenance issues
  • Make verbal or written reports to supervisors regarding any concerns and/or emergencies that may affect the safe treatment of consumers, the organization, or the safety of staff
  • Implement consumer’s treatment plan and treatment goals as required
    • Provides services primarily in the home of consumers to promote consumer health and a safe, stable, sanitary home environment
    • Provides services in the community with consumers which are related to individual treatment goals
    • Utilize community resources to assist consumer with personal/treatment needs
    • Provides health-related services, such as preparing accurate dosage of medication, administer/observe  self-administration of oral medications, and administer first-aid when necessary
  • Assist consumers during meal preparation, housekeeping responsibilities, personal hygiene, grooming and other activities as needed.
  • Coordinate client schedules.
  • Communicate and collaborate with all related treatment team members (i.e. treatment, guardian, management, administrative, etc.)

Required Skills

  • Able to meet deadlines and work under pressure
  • Excellent verbal, analytical, organizational, and written skills
  • Must be able to exercise sound judgment

Qualifications and Education Requirements

  • Working telephone
  • Possess a valid Michigan driver’s license
  • Pass all regulatory checks
  • Possess reliable personal transportation
